Register

Every SDMC student must re-register every year - this is our principal means of updating student and family information and of eliminating inactive registrations.  Registration is now open for the 2009-2010 season.  Registrations can be accepted through the electronic form below at any time during the 2009-2010 season.

SDMC registration is largely electronic, but there are two parts that involve paper:

1. UCSD Liability Waiver.  This form is required for every SDMC student, every year.  The form may be downloaded as a PDF document HERE.  Both the parent (or guardian) and the student must sign this form; however, please be sure to PRINT the student name at the top of the form.
2. Financial contribution. The electronic form requests a voluntary pledge of a donation to support SDMC; however, the actual contribution should be made by check.

Both Liability Waivers and Donation Checks should be delivered in person to the Director, or be mailed to:

Please observe the following notes in completing this online registration:

• Please use conventional capitalization in name and address fields
• Avoid abbreviations - we may not understand you
• Please remember to pledge a contribution that reflects the value of SDMC to your student.
• Please proofread your information before hitting the "submit" button.
